Great Catch winners help YNHH become a high reliability organization

Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

Inpatient

Carlos Gonzalez, environmental services technician, was recognized for attention to detail, practicing 200% accountability and performing a self check using STAR (Stop, Think, Act, Review). While working on the Geriatric Medicine unit (Celentano 3), he saw a patient about to fall off a chair. Gonzalez called for help, ran over and helped ease the patient to the floor as staff arrived. His quick actions prevented serious injury to a high-risk patient.

Outpatient

Helene Sitkiewicz, assistant chief technologist, Diagnostic Radiology, was recognized for practicing a questioning attitude. A patient scheduled for a chest CT scan said he had a certain type of port that had been placed during a previous clinic visit to inject contrast material for the exam. Sitkiewicz checked the documentation and found that the patient had a different type of port than he had indicated. Her attention to detail saved the patient from a potential injury.
