Great Catch winners help YNHH become a high reliability organization

Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

Inpatient

Rosemary Minore-Carbone, RN, Heart and Vascular Center PICC, was recognized for practicing a questioning attitude and stopping the line to prevent an unnecessary repeat procedure on a patient. The patient had arrived in the Pediatric Emergency Department for a new PICC line placement. After the new line was inserted, a post-procedure X-ray indicated it was in the incorrect location and should be pulled. Minore-Carbone noticed the child was not positioned correctly for the X-ray and recommended a second X-ray, which showed the PICC line was in the correct location.

Outpatient

Sheila Brooks-Ross, radiology technologist, Diagnostic Imaging, was recognized for practicing a questioning attitude and sharing her concerns during a GI procedure. Her “internal smoke detector” went off in relation to the anatomical location where the GI team was injecting contrast. Brooks-Ross escalated her concerns twice during the procedure and her cross-check was accepted by the physician. Thanks to her use of CHAMP behaviors, the patient received the correct exam.
