Great Catch winners help YNHH become a high reliability organization

great catch june 2018

Maureen Arnold; Lina Klaus, RN

Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

Inpatient

Maureen Arnold, patient care associate, practiced a questioning attitude and validated and verified as she was preparing a room for a patient admission. Hearing a hissing sound from the oxygen gauge, she escalated her concerns to the unit patient service manager. The gauge was found to be defective, delivering oxygen even though it was turned off. Because of Arnold's attention to detail, oxygen gauges throughout the hospital were checked and replaced in the East Pavilion.

Outpatient

Lina Klaus, RN, Temple Medical Center, was recognized for attention to detail and speaking up for safety when she noticed a surgeon draping the incorrect eye while setting up a procedure on a patient. Klaus stopped the line and provided a cross-check to the surgeon. The team verified the surgical site and the procedure was performed on the correct eye.
