Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

Inpatient

Aaron Simmons, environmental services associate, was recognized for attention to detail, practicing a questioning attitude and demonstrating 200% accountability when servicing a bathroom in the York Street Campus Emergency Department waiting room. He accidentally triggered the assistance pull cord and realized that there was no sound. He went to the ED triage desk, verified that no alarm had sounded and called the Service Response Center to report the issue. Thanks to his use of CHAMP behaviors, Simmons prevented possible future harm to any patient or visitor due to the malfunctioning alarm.

Outpatient

Deborah Patton, RN, outpatient transplant nurse coordinator, was recognized for attention to detail, demonstrating 200% accountability and speaking up for safety. While reviewing the record of an end-stage liver-failure patient at the St. Francis clinic, she realized that one of the laboratory tests, which helps prioritize patients on the transplant waiting list, could not provide a result. Because the patient’s health was critical, this test was a priority. Patton escalated her concerns to the unit’s assistant patient service manager, who worked with the inpatient team to resolve the issue. Thanks to Patton’s use of CHAMP behaviors, the patient remained at the top of the waiting list and underwent a successful liver transplant two days later.
