Great Catch winners help YNHH become a high reliability organization

Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

Inpatient

Steve Ghiroli, lead pediatric echo sonographer, and Tylisa Johnson, clinical services assistant, Heart and Vascular Center echocardiography, were recognized for attention to detail and practicing a questioning attitude. The day before a pediatric patient’s biopsy surgery an order was entered for an adult echocardiogram. Johnson noticed that the patient’s age did not match the order and expressed her concerns to Ghiroli, who contacted the physician and had the order changed so the patient could be appropriately evaluated for surgery.

Ambulatory/outpatient

Maria Boguniecki, nuclear technologist, Heart and Vascular Center, Guilford, was recognized for attention to detail when she noted abnormal images on a scan she was performing. An EKG was subsequently performed and the cardiologist confirmed a condition requiring immediate treatment.
