Great Catch winners help YNHH become a high reliability organization

Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

catch1

Lajamia Banks; Nanci Coleman, RN

Lajamia Banks, registration associate, Central Scheduling, and Nanci Coleman, RN, Haelen Primary Care Center, were recognized for attention to detail, practicing a questioning attitude and escalation of concerns. Banks took a call from a patient who mentioned feeling unwell after inadvertently getting lidocaine patch gel in their mouth. Banks escalated her concerns to Coleman, who recognized the patient’s slurred speech as a possible sign of stroke. After conducting a brief stroke screening by phone, Coleman determined that a call to 911 was warranted. The patient was transported to the Emergency Department and diagnosed with a transient ischemic attack.

catch2

Gabriella Saraniti, Heart and Vascular Center interventional technologist, was recognized for attention to detail and speaking up for safety. During a vascular procedure, Saraniti stopped the line and alerted the provider that the three-way valve on the patient’s central venous access device was turned incorrectly, allowing air into the line. The physician immediately corrected the three-way valve directional flow and thanked Saraniti for speaking up for safety and preventing a potentially harmful event.
