Schedule your onsite vaccination

Yale New Haven Health will soon launch its annual flu vaccination program to help protect employees and keep patients and families safe.

Beginning in early October, when seasonal flu activity generally begins, YNHHS will provide vaccinations to employees throughout the health system. All employees and physicians, whether working onsite or on a hybrid or remote schedule, must comply with the flu vaccine policy as a condition of employment across all YNHHS organizations.

Employees at Lawrence + Memorial and Westerly hospitals and staff at Health at Home – Southeast must receive a flu vaccination or complete a declination form.

Eligible employees may apply for a medical or religious exemption. Those working fully remotely with no onsite responsibilities may request an annual conditional deferment for flu vaccination.

Onsite flu vaccinations will begin at locations throughout the health system the week of Oct. 6. The deadline to comply with the flu vaccination program is Monday, Dec. 1, 2025.

Employees must schedule an appointment for an onsite vaccination by:

  • Logging into Infor
  • Selecting YNHHS Employee in the upper left
  • Under Employee Information, selecting Employee Health
  • Clicking Get Your Flu Vaccine to schedule an appointment

Volunteers, students, travelers, vendors and non-employed licensed independent practitioners may schedule an appointment through ynhhs.org/GetVaccinated, or by visiting https://fluvaccination.ynhh.org/.

Alternatively employees may get vaccinated by their primary care physician, at a YNHHS outpatient pharmacy or at a participating retail pharmacy in the CVS Caremark vaccination network or a CVS Minute Clinic.

Visit Infor; Employee Health; Flu Validation/Exemption for details about vaccination options, including how to submit documentation of vaccination by a primary care provider or participating pharmacy, as well as declination and conditional deferment forms.

Proof of vaccination (if received outside of the YNHHS onsite flu vaccination fairs), exemption requests, conditional deferments, and declination requests must be submitted by Dec. 1, 2025.

Additionally, all employees must complete the online flu module on the Infor Learning Management System by Dec. 1. This includes employees who have declined the vaccination and those working remotely.
