Jane Coffee, APRN

Jane Coffee, APRN is a board certified adult/gerontology primary care nurse practitioner specializing in geriatrics and palliative care. She is passionate about ensuring homebound patients have equal access to comprehensive and effective primary care. Her clinical practice goals are to reduce symptom burden, relieve pain and discomfort, and enable her patients to maximize their quality of life. YNHH/NEMG’s unique geriatric home based primary care program enables Ms. Coffee to provide both urgent and routine appointments within her patients’ home environment. She treats a wide range of chronic conditions typical within the geriatric population. Her aim is to ensure her patients and their families feel comfort and relief and are able to continue to live full lives within their home setting. Ms. Coffee graduated from Yale University School of Nursing in 2021. Prior to completing her APRN, Ms. Coffee worked as a palliative care and hospice licensed clinical social worker and registered nurse at the VA Medical Center for over ten years.
