Earn premium credit with new Personal Choice Activities
New “Personal Choice Activities” are part of this year’s Yale New Haven Health Know Your Numbers Plus (KYN+) 2025 program, giving employees more opportunities to earn up to $500 toward their 2026 YNHHS Medical Plan premiums. This year, employees may choose from familiar favorites and options such as breast and colon cancer screenings and other activities that support their health.
The annual KYN+ well-being incentive program launches May 1. It supports employees’ physical, social, emotional and financial wellness and can provide key insights into current health status and future risk. The program consists of a biometric screening and the Personal Choice Activities.
To earn credit, employees must first register on the Personify Health (formerly Virgin Pulse) platform on the THRIVE intranet site or through HRConnect, under Health and Wellness, Quick Links. Employees may also download the Personify app. More than 17,000 YNHHS employees are already enrolled in Personify (through the former Virgin Pulse), including everyone who participated in Know Your Numbers in 2024. (Personify log-in credentials are the same as they were for Virgin Pulse.)
The biometric screening (which measures blood pressure, cholesterol, height, weight and body mass index) is required to receive $300 credit toward the medical plan. Onsite screenings will be available June 2 - Aug. 15. Employees may schedule appointments through Personify or have their screening as part of a preventive care visit with their primary care or Ob/Gyn physician. Any credit earned by completing Personal Choice Activities will remain “locked” until a biometric screening is completed.
Options for Personal Choice Activities include various physical activities, dental visits, a health assessment, volunteering in the community and more, and can earn you an additional $200. The table lists eligible activities and their credit value. Biometric screening, onsite or with a PCP, and all eligible Personal Choice Activities completed between Aug. 26, 2024, and August 22, 2025, can be submitted for premium credit.
All KYN+ activities must be completed and documented in the Personify Health portal by Aug. 22, 2025.

* YNHHS employees at L+M or Westerly hospitals or Health at Home Southeast who are enrolled in the High Deductible Health Plan (HDHP) are eligible to complete the Online Health Assessment, a questionnaire that evaluates health status, risks and habits, to earn $50 toward their Health Savings Account for 2025. HSA dollars will be deposited into your account in October 2025.
Additionally, employees who elect the YNHHS Medical Plan for 2026 and complete a biometric screening will earn a $300 credit toward the 2026 Medical Plan premium. To receive the maximum $500 premium credit, employees must complete a biometric screening and Personal Choice Activities.
