Post-Acute Care

Many people may need additional help recovering from surgery, an accident or an illness. At Yale New Haven Health, we want you to continue to receive the care you need during your recovery. Depending on the intensity of the care you require, treatment may include a stay in another facility, outpatient therapy or home care. We care about your total care and want to make sure that you get the healthcare services you need, when and where you need them. That’s why we created the Yale New Haven Health Coordinated Care Network.

Yale New Haven Health has developed a Coordinated Care Network of home healthcare agencies and skilled nursing facilities based on performance criteria that include length of time taken to get patients back to a reasonable level of function as well as the Centers of Medicare and Medicaid Services Star Quality rating system. These facilities meet regularly with Yale New Haven Health to participate in quality improvement efforts designed to improve your care and experience.
