Yale New Haven Health and Yale Medicine now offer Connected Care: Manage My Health, a remote patient monitoring program designed to support your care between office visits. Using easy-to-use devices, you can check your vitals at home while staying connected to your care team every step of the way.
What is Connected Care?
Remote patient monitoring helps you stay connected with your clinician. Using simple devices, you can measure important health information at home. Your care team checks these measurements to:
Achieve good control of your health condition
Catch health changes early
Reduce trips to the hospital
Better manage ongoing health conditions
Help you feel better day-to-day
Who Can Benefit?
This program is currently designed for patients with:
Type 2 diabetes
High blood pressure
Certain heart conditions like cardiac insufficiency or advanced heart disease commonly called heart failure.
How It Works
Getting Started Your clinician will recommend the program for you if they think it could have a positive impact on your health. You will then receive a call or email to tell you more about the program and enroll you in the program if interested. The program is offered by our partners, Cadence, a leading healthcare company specializing in remote monitoring. You'll receive an easy-to-use monitoring device that will connect into your medical record and enable the Cadence care team help you manage the health condition.
Daily Checks Take quick vitals readings from the comfort of your home as directed by your Care Team. For patients with Type 2 Diabetes that would be a glucose meter, and for patients with hypertension that would be a blood pressure monitor.
Automatic Updates Your readings are automatically and securely sent to your care team without you having to do anything extra.
Personal Support The care team will check in with you regularly during telephone visits. If any of your readings indicate a potential concern, the care team will contact you right away to help.
Common Questions
Does insurance cover this? In most cases, yes. Coverage depends on your individual plan, and most patients have little or no out-of-pocket cost. We are happy to help you understand any potential costs based on your coverage.
What device will I receive? Depending on your health needs, you may receive:
A blood glucose meter (for diabetes)
A blood pressure cuff (for high blood pressure)
A blood pressure cuff and a weight scale for a heart condition called cardiac insufficiency or heart failure
