This program connects patients to resources as well as personalized recommendations for vetted local support including financial assistance.

Get started by visiting ym-ynhhs.wellist.com or call 855-WELIST (935-5478) Monday – Friday, 9 am – 5 pm.

How do I sign up?

Visit ym-ynhhs.wellist.com or call Wellist’s Support Team at 855-WELLIST (935-5478). The call center is open Monday - Friday from 9am - 5pm ET and supports over 200 languages.

What should I expect?

You’ll have full access to the information, resources, and support you need all in one place. Create and access your account using the Wellist website or phone line to find out what patients have found helpful and create a plan that’s right for you.

Can my family access this program as well?

Yes. Every Yale Medicine or Yale New Haven Health patient and his or her immediate family has access to Wellist.

What type of information can I find on Wellist?

Wellist provides information about patient tools and clinical services, as well as health information about your diagnosis, treatment, recovery and more. You can also find a crowdfunding tool to coordinate social and financial support from loved ones.

How do I find the best resources for me?

A little information goes a long way. By entering what’s most important to you – whether that’s prioritizing resources that are low-cost, close to home, or most relevant to your health – you’ll get access to curated, personalized recommendations.

Can I get resources if I don’t live near my site of care?

Yes. You’ll have access to local resources as well as hundreds of national services. You can choose services close to home or close to your treatment center.

How much do these services cost?

You will find a mix of free, discounted, and self-pay offerings on Wellist. If you do not find a resource that meets your needs, please call 855-WELLIST (935-5478) for a personal search to see if there is another option that might be available.

I have friends who are asking how to help. Can I share my Wellist Support Plan?

Yes. Once you receive your personalized recommendations, you can share your Support Plan with family and friends. If they choose, loved ones can contribute to help pay for the services you need most. Each contribution goes directly into a PayPal account linked to your email address. Wellist does not take a fee.
