Caroline J. Coffee, APRN

Jane Coffee is a committed healthcare professional with 15 years of experience in geriatrics, palliative care, and specialized care for medically and psychiatrically complex populations. She began her career as a Licensed Clinical Social Worker (LCSW) within the Veterans Health Administration, gaining valuable experience in primary care, palliative care, geriatrics, and spinal cord injury clinics. Ms. Coffee then transitioned into nursing, working in both the VA’s post-acute long-term care setting and inpatient hospice unit, before advancing to a Nurse Practitioner role at Yale New Haven Health's Home-Based Primary Care program. In this capacity, she primarily cared for medically complex, homebound older adults. She now serves as an APRN for Yale New Haven Health at Home Hospice and Quotidian Health, LLC. Her research interests are focused on advance care planning discussions, ethnic and racial disparities in palliative care access, and the experience of existential distress among patients. Ms. Coffee is deeply committed to improving healthcare delivery for vulnerable populations and enhancing the quality of life for individuals facing complex health challenges. She holds a Bachelor of Arts from the University of California, San Diego, a Master of Social Work from Portland State University, a Bachelor of Science in Nursing from the University of Connecticut, and a Master of Science in Nursing from Yale University. Additionally, Ms. Coffee is an LCSW and holds certifications in hospice & palliative care nursing and gerontology.
