The lazy, hazy days of summer may be time for rest and relaxation, but don’t let the Know Your Numbers Plus (KYN+) program pass you by.
Yale New Haven Health’s annual, voluntary well-being incentive program supports employees’ physical, social, emotional and financial wellness. It can provide key insights into employees’ current health status and future health risk. The program offers benefit-eligible employees an opportunity to earn up to a $500 credit toward their 2026 YNHHS Medical Plan premium. This year’s program ends Aug. 22, 2025.
To participate, employees hired on or before Aug. 10 must first register on the Personify Health (formerly Virgin Pulse) platform, located on the THRIVE intranet site or through HRConnect, in the “Health and Wellness” section quick links. Employees may also download the Personify Health app to access the portal 24/7.
Participants must complete a biometric screening to earn $300 toward the YNHHS Medical Plan and may complete Personal Choice Activities worth an additional $200 in credit. Biometric screenings measure blood pressure, cholesterol, height, weight and body mass index. They are available onsite at all delivery networks through Aug. 15, 2025. To schedule an onsite screening visit Personify Health. Alternatively, employees may have their biometric screening as part of a preventive care visit with their primary care or Ob/Gyn physician.
Personal Choice Activities options include various physical activities, dental visits, a health assessment, volunteering and more. Qualifying Personal Choice Activities completed between Aug. 26, 2024, and Aug. 22, 2025, can be submitted for rewards. All KYN+ activities must be completed and documented on the Personify Health platform by Aug. 22, 2025. YNHHS employees at L+M or Westerly hospitals or Health at Home Southeast enrolled in the High Deductible Health Plan (HDHP) are eligible to complete an Online Health Assessment, a questionnaire that evaluates their health status, risks and habits, to earn $50 toward their Health Savings Account (HSA) for 2025. HSA dollars will be deposited into participating employee accounts in October 2025.
