Gregory Waterfield, lead tech, CT scan, won the Great Catch Award after identifying a significant patient identification error involving an extended-care facility resident. When the patient arrived from the Emergency Department, Waterfield verified the patient’s identification by checking the wristband and matching the patient’s information. The patient was hard of hearing and confused, limiting the ability to independently confirm identification. After the scan, the patient was returned to the ED. A short time later, the radiologist raised concerns that the imaging findings did not correlate with the patient’s prior studies and anatomy. Recognizing the seriousness of the concern, Waterfield continued to investigate the discrepancy and found the wrong patient had been transported from the extended-care facility.
