Know Your Numbers Plus 2026 (KYN+) is wrapping up! The YNHHS annual well-being incentive program gives employees the opportunity to earn up to $500 credit toward their 2027 YNHHS Medical Plan premiums.
While the program is open to all employees, rewards are available to benefits-eligible employees hired on or before Aug. 9, 2026. To participate, employees must first register on the Elevate platform (which replaced Personify Health). To access Elevate visit the employee intranet, go to Resources, then Benefits & Wellbeing. The bswift Elevate app is also available on the Apple App Store or Google Play Store.
All KYN+ components, including the biometric screening and well-being activities, must be completed and documented in the Elevate platform by Friday, Aug. 28, 2026.
Participants must complete a biometric screening to earn $200 credit toward the medical plan premium and may choose well-being activities worth an additional $300 in credit. Eligible activities are listed on the Elevate platform.
Biometric screenings can be scheduled through the Elevate platform and are available at various YNHHS locations through Aug. 28. Alternatively, employees may have their screening as part of a preventive care visit with their primary care or Ob/Gyn physician.
YNHHS employees at L+M or Westerly hospitals or Health at Home Southeast enrolled in the High Deductible Health Plan (HDHP) may complete a Health Assessment to earn $50 toward their Health Savings Account (HSA) for 2026. HSA dollars will be deposited into their accounts in October 2026. The Health Assessment is also accessible on the Elevate platform.
