The Ambulatory Pharmacy team marked a milestone in July with the enrollment of 25,000 patients in ambulatory pharmacy services that are embedded in various clinics, and through centralized referrals to Comprehensive Care Clinics (CCCs) across the health system.
CCCs are staffed with clinical pharmacists with advanced training who partner with patients, clinicians and care teams to optimize patients’ medication regimens and help them achieve their health goals. Pharmacists and pharmacy technicians work with the care team to ensure patients understand how to use their medications, monitor for side effects and manage drug interactions.
Pharmacy-led CCCs are currently at Yale New Haven, Bridgeport and Lawrence + Memorial hospitals. They serve patients who are on complex medications across primary care and other specialties, including dermatology, rheumatology, oncology, gastroenterology, pulmonology and cardiology.
