Overview

The Heart and Vascular Center offers a specialized program that focuses on chest pain and heart symptoms. People often feel frustrated when symptoms like chest pain, shortness of breath, and fatigue persist, even though tests show normal arteries. While these conditions are common in women, they can also happen in men.

Our Comprehensive Coronary Physiology Program is designed to identify the cause of one’s symptoms, explain it clearly, and create a personalized plan to help a patient feel better.

What makes our program different

  • The Comprehensive Coronary Physiology Program offered at Yale New Haven Hospital, specializes in care for patients with ongoing symptoms (e.g., chest pain, shortness of breath) without a diagnosis despite prior testing.
  • We use advanced coronary function testing ‒ a specialized form of heart catheterization procedure ‒ to identify the cause of a patient’s symptoms.

Research and innovation

  • Our program is closely connected to research, so patients receive care based on the latest science. Through research studies and clinical trials at Yale School of Medicine and across national research networks, patients have access to innovative treatments including coronary sinus reducer interventions. This involves an implant that slightly narrows the heart's main drainage vein, helping reroute blood to the heart muscle and reducing chest pain symptoms. Interventional Cardiology Treatments

Ischemia with nonobstructive coronary arteries (INOCA)

INOCA means the heart is not getting enough blood or oxygen, even though the main heart arteries are not blocked. This condition is common and can be treated. Common symptoms include chest pain, pressure, shortness of breath, fatigue, and symptoms with stress or exercise, or at rest.

Conditions that fall under INOCA include:

  • Coronary microvascular dysfunction (CMD). With this condition, the tiny blood vessels in the heart do not work the way they should. These vessels may not open enough to let blood flow and symptoms can feel just like a heart attack. The condition may not be picked up by routine cardiac tests. PET myocardial perfusion imaging or invasive coronary function testing can be performed to look at these small vessels.
  • Coronary vasospasm. Vasospasm happens when a heart blood vessel suddenly tightens or squeezes. This can reduce blood flow for a short time and may cause chest pain at rest, while sleeping, during exertion, or without any clear trigger. Stress, cold, or certain medications are common triggers in some people Because the artery looks normal most of the time during testing, this condition is often missed unless specific testing, called coronary function testing is performed.
  • Myocardial bridging. With this condition, a heart blood vessel runs through the heart muscle instead of on top of it. When the heart squeezes, the artery can be squeezed as well and may limit blood flow, especially during activity. Many people never know they have it unless they develop symptoms. Advanced imaging including CT scans and invasive coronary function testing are performed to see if a myocardial bridge is causing symptoms.

Myocardial Infarction with nonobstructive coronary arteries (MINOCA)

MINOCA means having a heart attack even though the arteries do not appear blocked. This can happen because of blood vessel spasm; small vessel disease; blood clots that dissolve quickly; or other causes not seen on routine tests. MINOCA requires careful evaluation and follow up care, including cardiac MRI or more invasive testing to understand the cause of the heart attack.

Spontaneous coronary artery dissection (SCAD)

SCAD happens when a tear forms inside a heart artery. It mostly affects women and often happens in people without traditional heart disease risk factors. It can occur during stress, pregnancy, or without warning. SCAD is different from typical heart attacks, and care should be provided by experienced specialists.
