Yale New Haven Hospital to co-host Community Health Forum

New Haven, CT (April 19, 2019) – Yale New Haven Hospital and Heathier Greater New Haven Partnership will hold a community public forum on Tuesday, April 23, 5:00 to 7:00 pm at the Wilson Library, 303 Washington Avenue in New Haven.

The purpose of the forum is to review the 2019 Community Health Needs Assessment (CHNA) along with the outcomes of local health and wellbeing needs that were identified in the recent Connecticut DataHaven Wellbeing Survey. The survey includes results of seven community conversations that were conducted in Greater New Haven.

The goals are to discuss the identified health priorities, which include promoting healthy lifestyles, mental health and access to care, in addition to gathering input on the Community Health Improvement Plans and Implementation Strategies identified for 2019-2022.

The Healthier Greater New Haven Partnership is a community health improvement coalition that includes Yale New Haven Hospital, Milford Hospital, local departments/districts of health, federally qualified health centers, and community and non-profit organizations serving the Greater New Haven Region.
