The Primary Care Center at Bridgeport Hospital is a Patient Centered Medical Home. We proudly care for a diverse group of patients and take a multi-disciplinary approach to address each patient’s individual’s needs. We assist patients for their primary care needs, after hospital discharge, for acute complaints, follow-up from a visit to the emergency department and for many specialty care needs.
