Yale New Haven Health - Health at Home, Lifetime Care at Home, provides non-medical care for you or your loved one in the comfort of your own home. Our personal and live-in care team helps clients with daily tasks on a variety of levels from helping run errands to 24-hour care. Non-medical caregivers do not help with clinical needs but provide crucial services to help our clients live their best lives.

In addition, data shows in-home care can help prevent re-hospitalizations.

 

Care Options

Personal Care

We can help make the necessary activities of daily living easier for our clients, enabling them to enjoy the comfort of their own homes. With a goal of keeping our clients happy and comfortable at home, our team members can provide aid for many tasks, including:


  • Bathing
  • Dressing
  • Walking
  • Eating
  • Toileting
  • Medication reminders

Activity Assistance/Companion Care

Sometimes a client is more than capable of taking care of their personal needs, but can greatly benefit from some assistance with daily activities and companionship. Our caretakers are trained experts in providing support for your loved one’s day-to-day activities including:


  • Cooking
  • Cleaning
  • Laundry
  • Chores
  • Shopping
  • Errands
  • Transportation
  • Medication Reminders

An activity assistant will be a great fit for clients who do not need “hands-on” care, and who require assistance for less than 5 hours a day, a few days a week. This option works best if you find yourself needing a ride to doctor’s appointments, wishing you could pop out to the store, or looking for help preparing meals. If you or your loved ones need more frequent help, or need help with more personal tasks such as dressing and bathing, you may want to consider a personal care assistant.

Live In or Around the Clock Care

For those needing full-time assistance, live in care provides consistent one-to-one care by a caregiver who lives in the client's home. Our live-in professionals provide services 7 days a week. People with memory impairment issues benefit from having one caregiver that they develop a relationship with and learn to trust.


Live in care services can include any of the personal care and activity assistance items listed above.


When a loved one cannot be left alone for even short periods, it is important to have an around the clock caregiver. Our around the clock care team is dedicated to meeting the needs of our clients any time of the day or night. This service entails hourly care that is usually scheduled in long shifts such as 8, 10, or 12 hours at a time. Typically, this requires multiple caregivers to cover all of the shifts.

Safety Monitoring

In addition to the watchful eye of our caregiver at home, we are able to help coordinate a variety on safety monitoring equipment.


Safety monitoring works well in conjunction with family and caregiver involvement.

Memory Care

Memory care can include hourly or live-in services for a client with memory-related illnesses. Clients typically feel more comfortable within their own home rather than in a memory care facility, and we can provide safe and consistent care for these individuals. We find that many times our clients are able to live safely and securely in their own home while still maintaining a sense of purpose and dignity.


Our experienced caregivers provide help with things like:


  • Personal care assistance (dressing, grooming, bathing, etc.)
  • Activities assistance (shopping, laundry, cooking, etc.)
  • Safety monitoring (medication reminders and more)

Covering Costs

Hiring in-home care can result in significant savings over time. A 2020 independent Genworth Financial survey shows the median yearly cost of care in Connecticut for a private room in a nursing home is $164,980 compared to a yearly cost of $136,875 for companion live-in care.

There are other ways to find savings with live-in care:

  • Search for every possible Connecticut home care funding source. The CHOICES program can help at 800-994-9422 or call the helpline at 866-474-5230.
  • Explore Veterans Aid and Attendance Pension, which may provide as much as $2,020 per month. For more details, visit www.veteranaid.org.
  • Use safety monitoring tools to substitute for some human help.
  • Consider an adult day center for daytime assisted living and supplement with private in-home care.
  • Work with a care team to match the home care service to the exact need. Too much or too little will cost more.
