As a part of our ongoing work to provide unparalleled value to the patients we serve, Yale New Haven Health has created the Clinical Redesign program to be an engine for innovation conceived and driven by front-line providers and staff. 

Clinical Redesign is grounded in a rigorous, replicable, data driven and outcomes-oriented process that combines cutting edge healthcare improvement and delivery science. Our rapid 90-day project cycles have clearly defined goals and performance indicators.

Who is Clinical Redesign?

Clinical Redesign is a physician led partnership between front-line clinical teams, performance improvement specialists, and sophisticated data analysts at Yale New Haven Health. Our front-line providers and staff identify areas for improvement and provide practical and clinical context, our performance improvement specialists contribute analytic and change management expertise, and our IT analytics team provides access to data and tools to measure the impact.
