PATH: Partnered Approach Toward Health

Everyone’s road to health is different. Partnered Approach Toward Health (PATH) provides personalized support from skilled nurses who partner with you and your doctor to guide you toward your health goals—at no cost to you.

What is Partnered Approach Toward Health (PATH)?

PATH is a free, 90-day program that provides personalized support for patients who may benefit from extra help managing their health.

If you’re eligible, our PATH team will reach out to you directly by phone. The program is designed to complement the care you already receive from your primary care provider (PCP) and includes:

  • Weekly phone check-ins with a specially trained PATH nurse
  • Coordination with your PCP so everyone stays informed
  • Help managing medications, appointments and day-to-day health needs
  • Connection to community resources like transportation, housing and food support
  • Documentation in your MyChart electronic medical record for continuity of care

Why you may receive a PATH call

Some patients are identified by Yale New Haven Health and your doctor as likely to benefit from short-term health support. If you receive a call from 203-688-7284, our team is reaching out because your care history suggests we can help make managing your health easier.

You don’t need to sign up — we’ll contact you if you qualify.

How PATH works

Over the course of 90 days, the PATH team partners with you to understand your goals, identify challenges and connect you with resources that make day-to-day health management easier.

  1. Welcome call: We introduce the program, listen to your goals and answer any questions.
  2. Weekly check-ins: Your nurse will call to review medications, appointments and progress.
  3. Care coordination: We communicate with your PCP and help you access community resources.
  4. Wrap-up: After 90 days, we review your progress and next steps.

Everything is voluntary and you can choose to stop at any time.

To help you feel confident answering or returning a PATH call:

  • We will never ask for your Social Security number, bank or credit card information

Can I sign up for PATH?

If you think you could benefit from PATH talk with your primary care clinician to see if you may qualify. Our team works with PCPs to connect with patients based on their health and care needs.

Does PATH replace my doctor?

No. PATH supports — not replaces — your existing care plan. Your PCP is kept informed of all updates.

How long does the program last?

PATH lasts 90 days and includes weekly calls with your nurse care manager.

Who is on the PATH team?

Your PATH nurse leads the program, supported by pharmacists, social workers and community health workers as needed.

Is there a cost?

No. PATH is a free benefit for eligible patients.

What if I don’t want to participate?

You can let us know at any time. There is no obligation to continue.

Is my information secure?

Yes. All information is documented securely in your MyChart electronic medical record.
