Great Catch winners help YNHH become a high reliability organization

Congratulations to the September Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

York Street Campus

Melanie Doody, patient care associate, Trauma Surgery (SP 6-4), was recognized for attention to detail and practicing a questioning attitude. Patient Transport arrived on the unit to take her patient to MRI, but Doody knew that beneath the patient's bandage-wrapped forearm was a metal piece holding a fracture. She stopped the line and escalated her concerns to the nurse about the metal piece; the nurse notified MRI and the test was cancelled.

Saint Raphael Campus

Gurinderpal Doad, PharmD, clinical pharmacist, was recognized for attention to detail and providing a cross check. During a code, Doad realized the sheets on the code cart for several medications reflected out-of-date dosing minimums and maximums. All code carts at the SRC were checked and found to have varying drug dosing sheets attached. This and other issues were escalated to Pharmacy management and operations, which updated all code sheets.
