Great Catch winners help YNHH become a high reliability organization

Inpatient

Sandra Wilson, RN, Heart and Vascular Center procedure area, York Street Campus, was recognized for mentoring with 200% accountability, practicing a questioning attitude and attention to detail. Before the arrival of a patient who was having a CT-guided biopsy, Wilson and a colleague discovered a discrepancy in a physician’s order. Wilson escalated her concerns to procedural leadership, who ensured that the proper order and consent were obtained for the correct procedure.

Outpatient

Ginny Tierney, APRN, and Jodi-Ann Oliver, MD, pediatric anesthesia attending, Yale New Haven Children’s Hospital Anesthesiology, were recognized for practicing a questioning attitude, attention to detail and mentoring with 200% accountability when caring for a young patient scheduled for elective surgery. During the pre-anesthesia visit, Tierney determined that a heart murmur was more severe than documented. She arranged for the patient to have an updated echocardiogram, which prompted postponement of the elective surgery.
