Inpatient

Katina Stowers, pathology clinical technologist, was recognized for attention to detail and practicing a questioning attitude while processing a specimen. The name on the specimen seemed familiar, so Stowers used STAR (Stop, Think, Act, Review) and cross-checked the pathology database. She found that a specimen with the same name had been submitted the previous day, and escalated her concerns to the nurse manager and clinical team, who discovered that the specimen had been mislabeled and belonged to another patient.

Ambulatory/outpatient

Melissa Grime, CT technologist, was recognized for practicing a questioning attitude, attention to detail and speaking up for safety. A patient arriving at Shoreline Medical Center in Guilford had been medicated by the referring provider due to an allergy to contrast material. Grime requested a delay on the scan to research the allergy history and discovered the patient had had a reaction during a previous scan. Grime escalated her concerns to the attending radiologist, who confirmed the patient should not have contrast, despite being pre-medicated.
