Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

Outpatient

Ellen Tilton, RN, Maternal-Fetal Medicine, Long Wharf, displayed attention to detail, practiced a questioning attitude and mentored with 200% accountability when communicating with a non-English-speaking patient about her medication. Using Language Services to communicate, Tilton noted that the patient had begun taking a thyroid medication approximately 10 days previously. After conducting a cross-check and contacting the outside pharmacy, she determined the patient was taking an incorrect medication. The pharmacy corrected the medication error, preventing a possible safety event.

Inpatient

Eun-Jin Lee, RN, Medical Intensive Care Unit (NP 9/10), displayed attention to detail and practiced a questioning attitude. She stopped the line for a patient who had been scheduled for an emergency CT scan after presenting with a change in one of his pupils. Lee escalated her concerns to the Ophthalmology department and learned the patient had received medication to dilate the eye. Her use of CHAMP behaviors prevented the patient from receiving an unnecessary procedure and led to a new process implementation.
