Compliance and documents

School placement coordinators must ensure all compliance requirements, documents, and orientation modules are complete for each student/instructor before attesting.

COVID vaccine mandate/Influenza requirement
The COVID vaccine and influenza vaccine are required for all students and instructors in our organization. Students/instructors may submit an exemption that has been approved by their college/university.

Network ID, Epic, and Pyxis Access
Network ID and Epic access will be requested by the school for returning instructors and students; Pyxis access and Epic access for new instructors will be requested by the hospital student placement coordinator.

Online Epic training modules at Yale New Haven Health System are required before using Epic. Courses must be completed individually.

Immunization and vaccination

All students coming on-site to any YNHHS facility must have demonstrated immunity or vaccinations.  Listed below are the acceptable records for vaccination (or immunity), and tuberculosis (TB) screening. All appropriate documentation is required to be kept on file for each student by the appropriate representative at the school.

Measles, Mumps, and Rubella (MMR)

  • Documentation of 2 doses of measles, mumps, and rubella (MMR) vaccinations OR 
  • Positive titers/blood tests for MMR 

Varicella (Chickenpox)

  • Documentation of 2 doses of varicella vaccination OR 
  • Positive titer (blood test for varicella) OR 
  • Clinician documented verification of past chickenpox (varicella) or herpes zoster (shingles) 

Hepatitis B

  • Documentation of any and all vaccination doses (typically 3) with dates and titers (blood test) results for hepatitis B, unless this vaccine series is officially declined

Tetanus/Diphtheria/Pertussis (Tdap) 

  • Documentation of any adult or adolescent dose Tdap vaccine within the past 10 years

Influenza 

  • Documentation of the seasonal flu vaccine or medical/religious exemption, is required for students onsite between October and March

Tuberculosis (TB) Screening 

  • Two TB negative skin tests 7 – 10 days apart within the past 12 months OR
  • If documentation of a single negative TB skin test within the past 12 months, must have a second negative TB skin test before coming onsite OR
  • A single TB blood test (i.e. Quantiferon, T-spot, or BAMT) within the past year

 

 

Instructor Orientation

Unit orientation - contact the Patient Service Manager of the assigned unit to schedule a visit. To find the current PSM, email the student placement coordinator of the specific delivery network

Pyxis -Pyxis access is requested concurrently with your Network ID and Epic account. Please note processing takes approximately 3 weeks and will be provided to your school
